Referral Criteria & Process

Young people aged 16 -1 8 can self-refer. Referrals are also accepted from parents, carers and any professional who works with a young person – including health, social care, education, third sector etc. Permission must be gained from the young person before the referral is made. A referral form must be completed which can be found at the following website : www.healthyyoungmindspennine.nhs.uk/eatingdisorders Young people under the age of 16 will need to be referred by a professional, please speak with your GP or another professional.
