The relevant paperwork accompanying the patient referrals should be a letter faxed to 020 3513 6105 The letter should be addressed to Dr Giovanni Galavotti/Jo Wallace and should include brief history including recent Body Mass Index (BMI), bloods, Electrocardiogram (ECG).
